This form can be used by anyone to express any concern, suspicion, complaint or inquiry.

The form also serves for submitting reports under Act No. 171/2023 Coll., on the Protection of Whistleblowers (hereinafter referred to as the “Act”). Reports submitted under this Act are handled strictly in accordance with the law. You can find more information here.
